I had the same problem.
In my case the cause was the fact that I use a non-standard ssh port (other than 22).
Adding the following to the command line or to SHELLINABOX_ARGS in /etc/default/shellinabox solved the problem:
--service=/s:user:grp:HOME:'ssh user-ssh@localhost -p 12345'
